Anyone recognize this dog? Abandoned at my shelter today

I know who the owner is, but I don’t have contact info for them. This dog was brought in as a stray, with some concerning circumstances. I really need history and behavior information for this dog, they will not get in trouble. We can have the owner sign the dog over, it’s just absolutely vital we get behavior information.
