Social services for elderly unhoused man

I encountered a man who needed some assistance because he tripped and fell. He is very elderly/frai and seems to be unhoused. I helpedhi.get his belongings to a small park area where he said he was sleeping. He seems to be cognitively with it. Is there someone with social services that can come to him and help? I know that may be too much to hope for. Who should I contact, if anyone.
