My Seattle health insurance company (Ambetter, Coordinated Care) sent me a fecal colon cancer test, and I was impressed that they were so proactive in worrying about my health.
It turns out that if you just get a colonoscopy as a SCREENING procedure then the insurance pays 100%. But if you take a test like this first, and a positive test then makes you get a colonoscopy, that colonoscopy is DIAGNOSTIC, and you will pay from your deductible.
The test has a high false positive rate, too.
